Methylcobalamin or cyanocobalamin: which form of B12 should you choose?

A
Adam DALI

On vitamin B12 labels, two names appear time and again: cyanocobalamin and methylcobalamin. One is said to be "synthetic," the other "natural," one "not as good," the other "better"… The reality is more nuanced, and it is worth taking a few minutes to consider it before choosing.

At Pure Lab Nutrition, we have chosen methylcobalamin for our Vitamin B12 Instant+. Here is what science says about the different forms, how B12 is absorbed, how to read a label, and which mistakes to avoid, with complete transparency.

  9. Summary

The 4 forms of vitamin B12

  • Cyanocobalamin: the form historically most used in supplements and medications
  • Hydroxocobalamin: a natural form, often used in injections
  • Methylcobalamin: one of the two "active" forms used directly by our cells
  • Adenosylcobalamin: the other active form

They all bear the name "cobalamin" because they share the same central structure organized around a cobalt atom. What changes is the small group attached to this core: a cyano, hydroxo, methyl, or adenosyl group. This is what gives each form its name and influences its stability.

Comparing cyanocobalamin and methylcobalamin

In 2022, a review published in the journal Molecules (Temova Rakuša et al.) reviewed the forms of B12 in foods, supplements, and medications.

Criterion Cyanocobalamin Methylcobalamin
Stability Very stable: the most stable form More sensitive, particularly to light
Activation Must be converted by the body Active form, requires less processing
Absorption Generally comparable for most people Generally comparable; some work suggests better retention
Specifics Contains a cyanide group, released during its conversion Form naturally present in the body

According to this review, some researchers recommend the natural forms (hydroxocobalamin, methylcobalamin, or adenosylcobalamin) for long-term use, in order to avoid the accumulation of cyanide in tissues, especially in smokers who are already exposed to cyanide through tobacco smoke. This is a precaution suggested by some authors, not an established risk for cyanocobalamin.

☀️ The question of light

The same review reminds us that all forms of B12 are sensitive to light, and particularly the active forms, methylcobalamin and adenosylcobalamin. Cyanocobalamin is significantly less sensitive to this degradation. For a methylcobalamin-based supplement, the packaging and storage are therefore of the utmost importance.

How is B12 absorbed?

The absorption of vitamin B12 is a multi-step process. In the intestine, it binds to a protein called intrinsic factor, produced by the stomach. This duo travels to the last part of the small intestine, the ileum, where most of the absorption takes place.

This main mechanism is effective, but it saturates quickly: according to the Temova Rakuša review, the absorbed portion goes from about 80% for a dose of 0.1 µg to 50% for 1 µg, then to less than 10% around 20 µg. There is also a second pathway, through simple diffusion, which is much less efficient: it allows for the absorption of a small part of the ingested B12 (around 1 to 2%), and about 1% for high oral doses (500 µg and more).

💡 What this explains

This is why B12 supplements often show quantities well above the 4 µg per day adequate intake set by EFSA for adults: a large portion is not absorbed. The review also specifies that high doses of vitamin B12 are considered safe, as no adverse effects have been identified.

What the body does with these forms

Here is the point most often forgotten: once in our cells, all forms of B12 are transformed into the same intermediate, then reconverted into the two active forms. The quantity of active B12 produced therefore also depends on our cells and our genetics, not just the form swallowed.

The review concludes that, for the majority of the population, the bioavailability of the four forms is considered comparable. However, it mentions studies showing, with cyanocobalamin, lower retention in tissues and greater urinary excretion than with the natural forms.

In other words: both forms are effective for correcting a deficiency. The differences mainly concern stability, activation, and certain specific situations.

So, which one should you choose?

🧠 Choose methylcobalamin if…

  • you prefer an active form that is naturally present in the body
  • you are a smoker
  • you are aiming for long-term intake, for example as part of a vegan diet

💊 Cyanocobalamin remains valid if…

  • you are looking for a very stable form, especially in fortified foods
  • your doctor has prescribed it for you

In all cases, vitamin B12 helps reduce fatigue and contributes to the normal functioning of the nervous system. And to preserve methylcobalamin, keep it protected from light and humidity.

🏷️ How to read the label properly

On a supplement, the form of B12 must appear in the ingredient list. Look for the exact name ("methylcobalamin," "cyanocobalamin"…) and the quantity per serving, expressed in micrograms (µg). Beware of vague phrasing like "natural vitamin B12" without further clarification. To learn more, read How to read the label of a dietary supplement.

Storage and conditions of use

Methylcobalamin is an active but relatively fragile form. To ensure its stability until taken:

  • Store it in a closed cupboard, away from direct light and heat
  • Avoid the bathroom: steam and temperature fluctuations weaken vitamins
  • Do not leave the bottle open for long periods
  • Respect the expiration date: past this date, the content may decrease, even if the product is not dangerous

These precautions also apply to other water-soluble B vitamin supplements. A poorly stored supplement gradually loses its effectiveness, without it being visible to the naked eye.

Common mistakes

  • Believing the form matters more than consistency: B12 taken once every three days, regardless of the form, will be less useful than daily intake
  • Demonizing cyanocobalamin: it is a stable and effective form, widely used
  • Leaving your supplement in direct sunlight, on a windowsill or in a car: active forms are sensitive to light
  • Replacing a prescribed treatment with a supplement without talking to your doctor
  • Choosing a supplement without checking the capsule shell: for a vegan, a vegetable capsule is essential

Frequently asked questions

❓ Is methylcobalamin "more natural"?

It is one of the forms naturally present in the body, unlike cyanocobalamin. In a supplement, the word "natural" therefore describes the molecule itself, not necessarily how it is obtained. Both forms remain effective.

❓ Can you switch from one form to another?

Yes. Since all forms are converted into the same intermediate in our cells, changing forms poses no particular problem. If you are following a prescribed treatment, simply talk to your doctor about it.

❓ Isn't 1,000 µg too much?

This quantity is explained by the way absorption works: at high doses, only a small fraction is absorbed. The Temova Rakuša review indicates that high doses of B12 are considered safe. In case of a specific situation (pregnancy, treatment, illness), seek advice from a healthcare professional.

❓ What about hydroxocobalamin?

It is also a natural form, often used in injections by medical prescription. The Temova Rakuša review highlights that it offers an advantage for people with a genetic disorder of B12 metabolism. In the United Kingdom, the NHS even indicates a preference for it over cyanocobalamin for injections, as it remains in the body for longer. In dietary supplements, cyanocobalamin and methylcobalamin are more commonly encountered.

❓ Is methylcobalamin B12 suitable if taking metformin?

Metformin can reduce B12 absorption by decreasing the efficiency of the intrinsic factor-dependent ileal transporter. Monitoring B12 status is advised during long-term metformin treatment. The methylcobalamin form, active upon absorption, may be relevant in this context — but the priority remains talking to your doctor.

❓ Can you take methylcobalamin in the morning on an empty stomach?

Yes, B12 is not a fat-soluble vitamin: it does not require fats to be absorbed. You can therefore take it with or without a meal. Just choose the time that promotes consistency.

Summary

1️⃣ Both forms are effective for correcting a deficiency
2️⃣ Cyanocobalamin is more stable, methylcobalamin is more sensitive to light
3️⃣ Methylcobalamin is an active and natural form, preferred by some for long-term use
4️⃣ Consistency matters more than the form
5️⃣ Keep your methylcobalamin away from light to preserve its effectiveness
